The Wish to Regress One of the persistent problems that had to be faced again and again in Gloria's treatment was her wish to regress. It was her wish to retreat from the world and from life and simply to be taken care of by her mother. She did not want to work, she did not want to be on her own. She did not wantto grow up and function as an adult member of society. From time to time she was quite explicit about this infantile wish. Her resistance to therapy was in part motivated by a powerful wish not to surrender her infantile and highly narcissistic wish. My therapeutic efforts made inroads on that wish and posed a threat to it. At several points in her career, Gloria made serious bids to get herself back in the hospital. She stopped taking medications, developed increasingly severe phobias and paranoid anxieties, and became increasingly bizarre and disruptive in her behavior at home. When I made it clear to her that she would have to go to a rather forbidding state hospital near her home and that she could no longer be my patient, she gave up her regressive bid and returned to her usual pattern of behavior. But the problem persisted. As therapy progressed the choice became more and more clearly defined for her. She had to choose between the regressive course and the course of increasing maturity and responsibility. The regressive course meant avoiding working in therapy, not gettting better, clinging to her symptoms—primarily phobias, anxieties, and paranoid ideas—and ultimately ending up a mental cripple for life. The progressive course meant working in therapy, giving up her fears and their incapacitating effects, taking charge of her life and starting to work to make it what she wanted. She had not been able to make any such decision; it is one that will be made, if at all, in the course of more years of therapy. But the issue is a primary one in her therapy.
